Prepare For Your Visit

Please make sure that the receptionist is aware of your arrival. At busy times, if you have been waiting past your appointment time, you may wish to remind the staff that you are there.

When You Arrive

The receptionist will collect basic demographic information when you arrive, for the purposes of record-keeping, billing, follow-up contact etc. This information is of course confidential and will only be used where necessary.

If there are special circumstances where information may not be released eg. due to ongoing legal issues, divorce or other private matters, then it is most helpful to have this in writing, if necessary from your lawyer, so that we can be clear who may/may not have access to your child’s medical information. This is best done as soon as you are aware of the situation, before any chance for inappropriate communication arises.

Be sure to bring your child’s Alberta Healthcare card with you. If your child does not have valid health insurance then it is important that you give us this information before you see the doctor. In that case, you are responsible for payment of the fee for the services provided. Other provinces have a reciprocal arrangement with Alberta, and we accept valid healthcare cards from these provinces. Quebec is an exception, as we cannot recover our fees billed to Quebec Healthcare, and must charge directly to the parent.

Please let us know if your personal information changes, in case we need to contact you for any reason.

Your First Visit

We ask that you arrive 15 minutes before the scheduled appointment if this is your first visit, to allow our staff to register your child, weigh and measure them.

If you have previous height and weight measurements which may be relevant, immunization records, previous medical reports etc. bring them along.

Medical records and History

It is a good idea if your child is on prescription medications for you to bring these with you so that I can check doses, dates etc.

For discussions concerning your child’s behaviour or learning at school, it is useful to see any written comments from teachers, report cards, and results of previous testing.
